Did you know cardiovascular disease is responsible for one in every three deaths in the United States? Heart disease and stroke are also leading causes of disability in our country causing serious illness and decreasing quality of life.
If you’ve had a cardiovascular event, such as a heart attack, coronary artery bypass procedure, or stroke or mini stroke, you may be eligible for MercyCare’s healthy heart program. This initiative empowers members to make healthy choices and improve their care by focusing on the most current treatment guidelines.
How MercyCare can help:
- Our case managers work with you and your doctor to coordinate your care.
- The nurse case manager can help you with specific problems or questions about your disease or condition and help you develop a plan to make necessary lifestyle changes.
- The nurse case manager can also help you coordinate your care and connect you with resources within your provider network and your community.
- With additional support from the nurse case manager, you can begin to take steps toward better health.
Who is eligible?
The healthy heart program is available at no cost to all MercyCare members who have had a cardiovascular event. It is designed to help you better understand and manage your condition.
