MercyCare offers you a Health Risk Assessment (HRA) to maximize your health and your quality of life. The information in your HRA is private and confidential and will not be shared with your employer or used to determine your insurance coverage or premium.
Your HRA will help determine:
- Heart health
- Cancer risk
- How old you are based on your current lifestyle habits
- Areas in which you are doing well
- What you can do to improve your health
Before you complete the HRA, it is helpful to know the following values:
- Height and Weight,
- Blood Pressure (diastolic/systolic),
- Cholesterol (Total, HDL, and LDL), and
- Hemoglobin A1C (HbA1C)
MercyCare Health Plans’ partnership with Mercyhealth provides members a complete continuum of health care services. We also offer access to additional contracted providers, specialties and services in outlying areas. Our members receive care from one of the top integrated health care systems in the nation at over 170 provider locations throughout southern Wisconsin and northern Illinois.
Awarded the Malcolm Baldrige National Quality Award in 2007—the nation’s highest presidential honor for quality and organizational excellence,—Mercyhealth is a vertically integrated health system with 70 facilities serving 26 communities throughout southern Wisconsin and northern Illinois. Mercyhealth sees over 1,200,000 patients annually, and employs approximately 4,000 partners, nearly 400 of whom are W2 partner physicians.
97% of Mercyhealth physicians are board certified and 100% are board eligible. This is a remarkable average—much higher than the national average, and higher than other area health care providers. Board certification is widely recognized by physicians, health care institutions, insurers and patients as an essential tool to judge that a physician has the knowledge, experience and skills to provide quality health. NOTE: With the exception of Members of Congress or designated Congressional staff, the health insurance Marketplace does not affect FEHB program enrollees. For additional information, click here to visit the US Office of Personnel Management’s (OPM) website.
