MercyCare Health Plans’ partnership with Mercy Health System provides members a complete continuum of health care services. We also offer access to additional contracted providers, specialties and services in outlying areas. Our members receive care from one of the top integrated health care systems in the nation at over 170 provider locations throughout southern Wisconsin and northern Illinois.

Wellness incentive program (2016)

MercyCare’s free, voluntary, employee health and wellness program is designed to promote a healthy lifestyle for you and your family. It is offered to all state of Wisconsin employees and their enrolled spouse or domestic partner insured by MercyCare Health Plans. When you choose to participate, MercyCare offers you a Health Risk Assessment (HRA) to maximize your health and your quality of life. The information in your HRA is private and confidential and will not be shared with your employer or used to determine your insurance coverage or premium.

Your HRA will help determine:

  • Heart health
  • Cancer risk
  • Nutrition
  • Fitness
  • Stress
  • Safety
  • How old you are based on your current lifestyle habits
  • Areas in which you are doing well
  • What you can do to improve your health

Why should I participate?

The best reasons are to improve your health and your quality of life.  In the process, you will learn your health status and receive valuable wellness resources.

MercyCare is also happy to offer you:

  • A $150 incentive, paid once we receive both your biometric screening results and your completed HRA
  • If it is necessary to get new testing for your cholesterol or blood sugar, these are provided without any out-of-pocket expenses, when arranged by MercyCare.
